#traumatuesday brings us to a type of #fracture known as PILON, which is the French word for "pestle" an instrument used for crushing/pounding.
Pilon fractures are classified based on the number of Tibia (large shin bone, located on the left of this photo) fractures and where the fracture occurs. As an example through the metaphysis or number of fractured peices.
The Mechanism of Action (MOA) for truama tends to be due to high impact crush type truama. For example fall from heights.
The Fibula (smaller shin bone, located on the right in this photo) is also broken.
Many Pilon fractures require surgical intervention to correct the break and provide long term stability of the distal aspect of the tibia and fibula.
This patient fell from a pull up bar, a few feet from the ground. They completely dislocated thier ankle mortise joint which had to be manually reduced at the emergency room. The orthopedic surgeon then surgically repaired the joint by placing a few screws and plates to assist with bone healing and long term stability.
The patient was in a soft cast for approximately 6 weeks then an air cast boot for another 3.
With appropriate rehab, this patient was able to regain approximately 90% of thier mobility! They are able to run, lunge and squat with minimal limitation.
As an aside the #hardware will most likely stay as it does not impact the patient negatively
